2025-2026 Game Report Form

If a player or coach is disqualified at any level of play, the referees must send a Game Report to the Director of Conduct and Integrity. Game Reports must be completed within 24 hours of the end of the game.

Metro (High School) requires that a Game Report be filed for any Technical Foul or Unsportsmanlike Foul.

The Director of Conduct and Integrity may follow-up with you if more information is required.
